Qualification: The following must be submitted for the claim: A legible copy of the owner's signed original bill of sale or purchase contract. A copy of the physicians statement identifying the specific reasons for mobility assistance. A copy of the Mobility Assistance Claim form. A copy of the purchasers current drivers license. A copy of the current registration or title. A legible copy of the paid invoices by the original owner for the mobility assistance equipment installed by a qualified installer.

Eligibility: Only the original owner of the new and unused vehicle is eligible. Current owner's vehicle must be titled/registered under their name on the title or registration and must match the name on the purchaser's current driver's license, or the address on the title or registration must match the address on the purchaser's current driver's license or other proof of residency (i.e. prior month's utility bill in name of purchaser at current address, current property tax bill in name of purchaser at current address).

Loyalty Reward Program

Qualification: The Mazda Loyalty Reward purchase incentive has no cash value and must be applied within the lease as a capital cost reduction and as a purchase as a rebate. Military Appreciation Incentive Program

Qualification: There is a limit of two (2) purchases per person per year for the Military Appreciation Incentive. The name on the RDR has to match the name on the contract or lease agreement. One copy of each of the following is required for redempttion and must be kept in the sales jacket at the dealership. 1) Contract or buyers order 2) Documentation that clearly reflects active, reserve, veterans/retirees within 2 years of separation status. Leave and earnings statement, Delayed Entry/Enlistment (DEP) Form, Veterans/Retiree DD-214 Form 3) A copy of the purchaser's current driver's license.This incentive can only be stacked with one other private offer Owner Loyalty. Transferable within household only.

Eligibility: Active Duty Members, Reserves, Retirees and disables veterans within 2 years of separation or retirement of the US: Air Force, Army, Navy, Marines, National Guard, Space Force, Coast Guard Family including spouse and children are eligible but must live at same address.
