The two important rules to remember when appealing a health insurance claim denial are: (1) Document everything in writing, and (2) keep all documents in a secure, central place.

Following are some tips to appealing a health insurance claim denial:

  • As with illness, an ounce of prevention is worth a pound of cure. Ask your doctor to document and explain all recommendations; keep the documents in a central folder and know which procedures will be covered by your insurance prior to undergoing treatment.
  • Keep a record of the dates, times, and information discussed in all correspondence regarding the insurance claim.
  • Do not spend time disputing information over the phone with customer service personnel. If they are unable to resolve the issue within a few minutes, politely ask them how to submit a written appeal.
  • Send your appeal via certified mail and request a return receipt. Your appeal should include all relevant information.
  • Include all supporting documents, including medical records, reports, and letters from your physician certifying medical conditions.
  • Don't be too quick to obtain the services of an attorney. You'll likely be able to handle the appeal yourself, at no cost. If you can't, contact a patient advocate group for advice. Your physician might provide guidance, also.
  • If your appeal is approved, retain copies of the documentation. If your appeal is denied again, consider filing another appeal or consulting an attorney. Persistence sometimes pays.
